Intrapartum Fetal Surveilance
The document is a lecture note on Intrapartum Fetal Surveillance by Professor Lanka Dasanayake, a Professor of Obstetrics & Gynaecology at the University of Ruhuna, Faculty of Medicine in Sri Lanka. The lecture discusses the management of different cases during labor, emphasizing the importance of timely detection of fetal hypoxia and the prevention of perinatal/neonatal morbidity and mortality. It outlines various methods of fetal surveillance, including intermittent auscultation, electronic fetal heart monitoring (CEFM), fetal blood sampling (FBS), fetal lactate testing, fetal pulse oximetry, and ST waveform analysis (STAN). The document also describes different types of fetal compromise during labor, such as acute fetal hypoxia, sub-acute fetal hypoxia, gradually evolving hypoxia, and chronic hypoxia. Additionally, it provides a physiological interpretation of CTG (cardiotocography) and discusses the evidence for physiological interpretation of CTG, FBS, and combined fetal monitoring with ST-analysis (STAN). The lecture note also includes references and a post-test with three cases for management consideration.
